Eligibility:
AFFCF can consider funding for up to two months at a time ongoing for a social club or group such as Scouts, Brownies, 4H, School Science Club, etc. AFFCF can consider one club or group per child at a time. If a uniform or equipment is required for the club, please submit an application using the category Athletic Equipment/Uniforms. Clubs/Groups requests need to be for a business/organization whose information, services and enrollment/use of services are accessible to the public.
Documentation:
Documentation is required beforehand. The required supporting documentation must show the name of the club/group, length of membership and total cost for the club/group and to whom the payment should be made.
Payment:
Once a request is approved, a check payable to company can be mailed or an e-mail will be sent so then the purchase can be made and receipt(s) submitted for payment. The maximum AFFCF can consider funding is $125 per month. Frequency: up to 2 months at a time ongoing.
Other Resources:
- Opportunity 4 Kids – Funds extracurricular activities for children in Arizona whose household income is 150% of federal poverty guidelines. Can submit for up to 3 months at a time (if relevant). Check is sent directly to the service provider (not the requester). $1500 max per child/lifetime for all scholarship requests.
