Eligibility:
AFFCF can consider glasses if documentation can be provided that displays the AHCCCS will not fund the request. A specific, individual need for the youth is required for consideration.
Documentation:
Documentation is required beforehand. Documentation must show the vendor name, youth name, items requested and the full cost.
Payment:
Once a request is approved, a check payable to company can be mailed or an e-mail will be sent so then the purchase can be made and receipt(s) submitted for payment.
Other Resources:
- Navajo County Friends of CASA – Able to fund glasses for foster youth adjudicated or placed in Navajo County (don’t need to have a CASA).
