Eligibility:
- AFFCF can consider funding ethnic hair care services for protective hair styles such as braids, extensions, straw sets, twists, etc.
- **Haircuts and general or in-home hair care products, e.g., shampoo, conditioner, brushes, combs, etc., are excluded.**
- Race/ethnicity is a mandatory field to be completed in the application.
- Requests need to be from a business/company whose information, services and enrollment/use of services are accessible to the public.
Documentation:
The application description needs to describe the service(s) being requested. Race is a mandatory field in request and Case Manager response re: youth race is required beforehand and if the requested services are being sought from an individual, AFFCF needs to receive a W9 for tax purposes.
Payment:
Once a request is approved, a check payable to company can be mailed. Requests for reimbursement cannot be considered. If the vendor is an individual, an IRS form W9 is required. The maximum amount AFFCF can consider funding for ethnic hair care services is $125/month. Frequency: Monthly ongoing
