If you are interested in using CogniTutor (*note – the documentation required for CogniTutor is different than the instructions on the Childhood Activities page. We are in the process of evolving ALL of our tutoring vendors to this model; please follow instructions below if you want to use CogniTutor):
- Go to CogniTutor website and fill out the form www.cognitutor.com/get-a-tutor
- Check the box indicating the student has a case manager.
- In Case Manager Contact Info section – If the person submitting the AFFCF funding request is different from the caregiver, complete the “Who will the referrals and authorizations come from” fields.
- Select Arizona Friends of Foster Children Foundation AFFCF as the funder.
- Leave all other fields in this section blank.
- In the “Additional Comments” section (after “How Did You Hear About Us?”), write Childhood Activities request.
- Once CogniTutor receives the form, Ann or Aline will reach out to welcome you to CogniTutor and provide an Estimate of Services form to submit along with your tutoring need documentation (a recommendation on school letterhead or from school email address from the youth’s teacher(s) that tutoring is recommended and includes teacher’s contact information and subject(s) to be tutored OR specify whether the youth has an IEP and/or 504 plan) with your Childhood Activities tutoring request. You only need these 2 pieces of documentation and do not need to submit a Confirmed Tutoring Services form.
- Once you receive an email from AFFCF with an approval for tutoring funding, please forward that approval email to CogniTutor ([email protected]) so they have the request number which they need for submission of an invoice to AFFCF for payment and let them know the best way to contact you to schedule tutoring. CogniTutor will then contact the caregiver to schedule tutoring.
- AFFCF will fund a maximum of 8 hours of tutoring per month. For renewal of tutoring funding, please follow the above steps and submit another funding request, optimally 2 weeks before the 8 hours is used, to allow adequate time to process your request (7-10 business days is our standard processing time). AFFCF will not pay for tutoring services provided without prior request submission and approval.
