Barbara Polk Healthcare Scholarship

Financial aid  for undergraduate and/or graduate degree-seeking students studying healthcare fields

ABOUT THE POLK SCHOLARSHIP

The Barbara Polk Healthcare Scholarship, named in memory of one of AFFCF’s founders, is a scholarship available for four semesters for undergraduate and/or graduate degree-seeking students studying healthcare fields.

It is structured to provide the most support (financially) to students who are studying upper-level coursework of undergraduate study or who are in graduate medical or healthcare programs, periods when students may not be able to work due to clinical rotations, demand of coursework, etc.

Students who are studying healthcare fields will see a prompt to apply for the Barbara Polk Scholarship after they complete the AFFCF scholarship application. Eligible students will be:

 

    • Those who reached (or will be reaching) age 18 as wards of the state of Arizona or in tribal foster care in Arizona
    • Those who are under the age of 25
    • 3.0 minimum cumulative GPA required
    • Full-time enrollment required
    • Studying an undergraduate or graduate healthcare degree
    • One letter of recommendation from a teacher or professor of a health-related or science class required

Whether you are currently in foster care, were in foster care, have aged out of foster care, or are someone who works with youth in foster care, this education guide will provide information about the resources that are available to help young adults reach their educational goals.

FIRST-TIME APPLICANTS

Complete the Post-Secondary Scholarship Application and include the following:

  • One letter of recommendation from a teacher or professor of a health-related or science class required
  • Student’s most recent transcript (3.0 minimum cumulative GPA required)
  • Student’s FAFSA Summary Aid Report (SAR)
  • Student’s upcoming class schedule (full-time enrollment required)

Spring 2024 Semester Application Period:
November 15 – December 15, 2023

*Please see below for additional information about completing the Barbara Polk Healthcare Scholarship application if you are already a member or graduate of the AFFCF Scholarship program.

RENEWAL STUDENTS

Each semester, renewal students (who have already been accepted into the AFFCF Scholarship program) must provide AFFCF with the following to be eligible to renew their scholarship.

  • Student’s unofficial transcript displaying grades and cumulative GPA from most recent semester
  • Student’s upcoming class schedule (minimum enrollment of six credit hours required)

 

Spring 2024 Semester Renewal Period:
December 15 – December 31, 2023

*If you are already a member or a graduate of the AFFCF Scholarship program, you have already completed the AFFCF Scholarship application in the past. For that reason, please put “N/A” for the personal essay sections on the AFFCF Scholarship application. Also, you do not need to upload the letter of recommendation that the AFFCF Scholarship application requires, only the recommendation from a teacher or professor of a health-related or science class for the Barbara Polk scholarship application. You will need to complete the Barbara Polk essay question found in that part of the application.
