TUTORING - FOCUS FORWARD

Eligibility: Up to $560/month (8 hours of tutoring at $70/hour max). Student is to be aware of tutoring resources offered at their school and use those resources or explain how those tutoring resources aren’t optimum or available. If the school tutoring resources aren’t optimum/available, the student can get a tutor from CogniTutor or Einstein Tutoring (Flagstaff area only). CogniTutor offers virtual tutoring and may offer in-person tutoring depending on where you live and the subject(s) you need tutoring in. Einstein Tutoring is an option for students who live in the Flagstaff area and is in-person (Ex: campus library). If you want to work with a different tutoring vendor, please include Focus Forward request documentation detailed below.

Focus Forward tutoring requests don’t count towards the maximum number of Focus Forward-funded requests allowed per quarter and there is no maximum number of tutoring requests in a 12-month period.

Documentation:  Submit a course schedule and the Estimate of Services form the tutoring vendor provides you with your Focus Forward request for tutoring. Follow the directions in the hyperlink if you want to use CogniTutor or Einstein Tutoring – we have completed W-9s on file for each of them already and they will provide you an Estimate of Services form.

If you want to work with a tutoring vendor that is not CogniTutor or Einstein Tutoring, you must have that tutoring vendor complete a W-9  and you submit the completed W-9, a completed Estimate of Services form and your course schedule with your Focus Forward request.

You do not need to submit a budget for a tutoring Focus Forward request.

Payment:

CogniTutor or Einstein Tutoring will send AFFCF an invoice for tutoring services provided (up to the approved number of hours).

If using a different tutoring vendor, they must submit an invoice (use this template if needed) to AFFCF ([email protected] AND [email protected]) for the number of hours of tutoring provided (not to exceed the number of tutoring hours approved).

Please note: AFFCF will not pay any past due balances or for services provided prior to a Focus Forward request being approved. AFFCF reserves the right to deny a Focus Forward request for any reason or no reason at all. Thank you for your understanding and allied support.
